We collect only the information reasonably needed to provide care, manage appointments and billing, communicate with you, meet legal obligations, and operate or improve our services. We do not sell patient records or personal health information. You may ask to access or correct your records, subject to legal limits.

This policy explains how Physical Edge Physiotherapy Inc. ("Physical Edge," "we," "us," or "our") collects, uses, discloses, safeguards, and retains information in connection with: our clinic and health-care services; our website at https://www.physicaledgephysio.com; our online booking site at https://physicaledge.janeapp.com; and communications with our team, including website forms, email, telephone, and appointment-related messages.
For personal health information in our custody or control, Physical Edge acts as a health information custodian under Ontario's Personal Health Information Protection Act, 2004 (PHIPA).

3. Information we collect
- Identity and contact information: name, mailing address, telephone number, email address, date of birth, emergency contact.
- Appointment and account information: booking details, appointment history, reminders, communication preferences.
- Personal health information: health history, symptoms, assessments, diagnoses, treatment plans, clinical notes, exercise programs, referrals.
- Billing and insurance information: needed for payments and receipts and, when authorized, to submit insurance claims on your behalf. We do not offer direct billing: the insurer pays any reimbursement to you, not Physical Edge.
- Inquiry and communication information: website forms, email, calls, surveys.
- Website and device information: pages viewed, approximate location, browser/device type, interactions.
Our general website contact form is not intended for urgent concerns or detailed health information.
4. How we collect information
- Directly from you: when you contact us, book an appointment, complete an intake form, receive treatment, make a payment, or exercise a privacy right.
- From authorized people: a parent, guardian, substitute decision-maker, referring provider, or insurer.
- Through service providers: website host, booking platform, payment processor, analytics provider.
- Automatically online: through cookies and similar technologies.
5. Why we use information
We use personal information and personal health information to: assess your needs and provide, coordinate, document, and follow up on health care; maintain your patient record; schedule and manage appointments and send reminders; communicate with you; process payments, issue receipts, and when authorized prepare and submit insurance claims on patients' behalf; support quality assurance, training, safety, and operations; understand website use and improve performance and booking experience; prevent fraud or misuse; and comply with legal obligations.
6. Consent and your choices
We seek consent appropriate to the sensitivity of the information. You may withdraw consent for future collection, use, or disclosure by contacting us, subject to legal and professional limits. Appointment confirmations and reminders are operational communications. We send promotional messages only with required consent; you may unsubscribe anytime. Cookie choices can be managed through browser settings; blocking some cookies may affect site features.
7. When we disclose information
We disclose information only as reasonably necessary: within your care team; to other providers as permitted by law; for payment and insurance claims when authorized (we do not offer direct billing — reimbursement is paid to you, not Physical Edge); to service providers bound to protect the information; and for legal and safety reasons where required or permitted by law. We do not sell patient records or personal health information.
8. Service providers
- Jane: online booking and clinic administration. Jane's own privacy notice also applies to its platform.
- Wix and website services: hosting, security, and form delivery.
- Google Analytics: understanding website and booking activity in aggregate. We use cross-domain measurement so visits continuing to our booking site are recognized as the same session.
9. Processing outside Canada
Some providers may process or store information outside Canada, where it may be subject to that jurisdiction's laws. We use contractual, administrative, and technical measures appropriate to the service.
10. Cookies and analytics
Cookies support site functions and measure usage. Google Analytics may use cookies or similar identifiers. We do not send names, contact details, or clinical information to Google Analytics. You can opt out via the Google Analytics Opt-out Browser Add-on (https://tools.google.com/dlpage/gaoptout).
11. Safeguards and retention
We use reasonable administrative, technical, and physical safeguards, including role-based access, confidentiality obligations, staff training, secure systems, backups, and incident-response procedures. We retain information only as long as reasonably necessary and then securely destroy, erase, or de-identify it. Record-keeping duties may prevent immediate deletion of a patient record.
12. Your privacy rights
Subject to applicable law, you may ask to: access information in our custody or control; correct inaccurate information; withdraw consent for future uses; ask how information has been used or disclosed; or raise a privacy concern. We may verify your identity before responding.
13. Questions, complaints, and changes
Please contact our Privacy Officer first so we can address your concern. You may also complain to the Information and Privacy Commissioner of Ontario. We may update this policy as practices or obligations change; the current version will show its effective date.
