Main Findings: Prehab Helps Before and After Surgery

1. Better Function Before Surgery

People preparing for surgery with structured physiotherapy showed significant improvements in function compared with those who did not. This was especially true for:

  • Total Knee Replacement — Improved overall function and stronger knee flexor muscles.

  • Total Hip Replacement — Increased muscle strength and improved health-related quality of life before surgery.

  • Lumbar (Lower Back) Surgery— Less back pain and better quality of life before the operation. In fact, the evidence for back pain reduction was rated high-certainty — meaning the research strongly supports that prehab helps reduce pain before surgery.

Prehabilitation was consistently associated with improved muscle strength before surgery — especially around key muscle groups like knee flexors and hip abductors (important for stability and movement after joint replacements).

3. Better Quality of Life Before Surgery

People in prehab programs reported better health-related quality of life (HRQOL) — meaning they felt physically and mentally better equipped for the challenges ahead. This was seen in patients preparing for hip replacements and lumbar surgeries.

  • Improved function at 6 months after lumbar surgery 

These findings suggest that the benefits of prehab continue into the recovery period — particularly during the early and medium stages of postoperative healing, when strength and mobility gains are crucial.
